Hold Membership

Hold Request Form

This is not a termination request.

As stated in the membership agreement, clients have the option to put their automatically recurring monthly membership on hold for any reason.

Hold Policy
  • Hold requests are available in 30, 60 and 90-day increments.
    • They DO NOT count as a paid month in a membership contract.
  • Membership Holds will begin the day the form is received, or otherwise stated by the client.

  • Hold requests must be received no less than five (5) business days prior to the next billing date.
    • Requests received within 5 business days of the next billing date will be charged a $50.00 processing fee.
  • Upon expiration of the hold period, accounts will automatically reactivate and regular membership payments will resume.
    • All payments are non-refundable.
  • Should the decision to cancel the membership occur during the hold period, refer to the Membership Cancellation Policy.

I understand that my membership will be placed on hold the day this form is submitted or as I have requested.
I understand that by putting my membership on hold, I am extending my contract by the (30-60-90) day selected.
I understand that if I submit this hold request within 5 business days of the next billing date, I will be charged a $50.00 processing fee. All payments are non-refundable.
I understand that upon expiration of the hold period, the membership will automatically reactivate and regular membership payments will resume. All payments are non-refundable.
I understand that if I decide to cancel my membership during the hold period, I must refer to the Termination Policy.
